Hospital medical history as a medical, legal and administrative instrument in the new constitutional context

Given the relevance of the clinical history as a document that goes beyond the limits of the medical act itself, permeating with written evidence the entire trajectory of the continuum of care to the health-disease process of a person during his life and even after his death, the authors make a call to the professionals and directors of the hospitals regarding the validity of its importance as a medical, legal and administrative instrument in accordance with the new constitution.
