Eficacia de la ventilación no invasiva en los pacientes con fracaso respiratorio agudo
Abstract
Introducción. Con la Ventilación No Invasiva se evitan complicaciones relacionadas con el manejo de la vía aérea y el establecimiento de una vía artificial e incluye varias técnicas que consigue la ventilación alveolar sin intubar. Método. Se realizó un estudio pre-experimental (tipo antes-después), sobre la eficacia de la Ventilación No Invasiva en el tratamiento de pacientes con Fracaso Respiratorio Agudo ingresados en los servicios de Cuidados Intensivos de los Hospitales Provinciales de Ciego de Ávila en el período comprendido entre el primero de Marzo del 2005 y el 29 de Febrero del 2006. Resultados. Se estudiaron 45 pacientes, a los cuales se les practicó la técnica con máscara facial, de estos el 55.0 % eran masculinos con un promedio de edad de 49.5 años y 56.5 horas del proceder. La sepsis intraabdominal fue el diagnóstico 24.5% y la causa del inicio de la Ventilación No Invasiva fue el fracaso respiratorio hipoxémico causado por distrés respiratorio en el 51.1% de los mismos. Se utilizó presión de soporte presión positiva de expiración final (PEEP) + y presión positiva constante en la vía aérea (CPAP) de forma intermitente en casi la mitad de los pacientes, encontrándose mejoría significativa de los parámetros de monitoreo, incluso en los pacientes que requirieron más tarde ventilación invasiva, quedando probada la eficacia de la Ventilación No Invasiva. Conclusiones. Se logra mejoría ostensible de las alteraciones clínicas, espirométricas y gasométricas que genera el Fracaso Respiratorio Agudo de cualquier naturaleza, con pocas horas de Ventilación No Invasiva.Downloads
