Mortalidad en pacientes ventilados en la Unidad de Cuidados Intensivos del hospital de Morón.
Abstract
Introducción: En las Unidades de Cuidados Intensivos, la mortalidad es la mejor medida de evaluación de su rendimiento y desempeño. Muchos estudios se han encaminado a evaluar dicha variable. Método: Se realizó un estudio observacional retrospectivo para describir el comportamiento de variables relacionadas con la mortalidad en pacientes ventilados en el período de junio del 2007 a junio del 2011 en la Unidad de Cuidados Intensivos del Hospital General Provincial Docente de Morón. En este período se ventilaron 163 pacientes, de estos fallecieron 92. Se evaluaron 57 historias clínicas de las cuales se pudo obtener los datos para la realización del estudio. Resultados: La edad de los pacientes fue de 63.3 años, con un predominio de varones, con 9 días ventilados y 10.5 días en la unidad. El valor promedio de la puntuación de APACHE II fue de 20.3 puntos, lo que según cálculos pronósticos, prevé una mortalidad del 40%; sin embargo, la mortalidad observada fue de 56%. Predominaron los pacientes no quirúrgicos: enfermedad pulmonar obstructiva crónica e infarto agudo del miocardio. El 11.76% murieron por fallo multiorgánico. La neumonía asociada al ventilador apareció como complicación en el 45.6% de los casos, aportó una letalidad del 63.4%. El 52.6% de los pacientes recibieron una puntuación entre 19 y 24. Al calcular el riesgo de morir se encontró una probabilidad del 61.6%. Conclusiones: La mortalidad en el paciente ventilado es alta, el cálculo del riesgo de muerte constituye una herramienta útil.
