Miastenia grave ocular en el anciano. Presentación de un caso.
Abstract
La miastenia grave es un trastorno de la transmisión neuromuscular de origen autoinmunitario, producida por la disminución del número de los receptores de acetilcolina de la placa motora, se caracteriza por debilidad y fatiga muscular fluctuante principalmente de los músculos inervados por los pares craneales, su manifestación es evidente durante la actividad continuada y mejora con el reposo o tras la administración de medicamentos anticolinestarásicos. Se presenta un paciente de 79 años de edad, raza blanca, sexo masculino, con antecedente de padecer de diabetes mellitus tipo II para lo cual se controla con dieta, quien hace aproximadamente dos meses nota caída de los dos parpados, primero el derecho y después el izquierdo, además presenta dificultad para tragar, principalmente los alimentos sólidos y con el transcurso del día siente debilidad muscular, motivo por el cual asiste a consulta. Al examen físico se observa ptosis palpebral bilateral, parálisis del tercer par en su porción extrínseca. Se presenta este caso por su aparición en una edad y sexo no tan frecuente.Downloads
