Factores predictivos relacionados con la mortalidad por infarto agudo del miocardio en el hospital provincial de Ciego de Ávila. Año 2006.
Abstract
Se realizó un estudio descriptivo retrospectivo con el objetivo de conocer los factores más relacionados con la mortalidad por Infarto Agudo del Miocardio en el Hospital Provincial de Ciego de Ávila. Se tomaron todos los pacientes que ingresaron con el diagnóstico de IMA desde Enero–Junio de 2006 para un total de 73. Las variables sobre las que trabajamos fueron: Las formas de presentación clínica del IMA, la edad, la Diabetes Mellitus, la Hipertensión Arterial y el tratamiento trombolítico. La presentación típica predominó en el 93,1 % de los pacientes, siendo los que más se beneficiaron con la trombolisis (66,1 % vs 20,0%) y los que tuvieron una menor mortalidad en comparación con los que acudieron sin dolor toráxico (14,7% vs 40 %). Con la edad (70 años y más) aumentó la incidencia de la enfermedad y aumentó específicamente la incidencia de la clínica atípica del Infarto, siendo en los ancianos el grupo que menos recibió trombolisis (81,48 %) y por tanto el grupo de mayor mortalidad (75 %). Los diabéticos no mostraron diferencias significativas con los no diabéticos en cuanto a la forma de presentación y al tratamiento, sin embargo su mortalidad fue considerablemente mayor en relación a los no diabéticos (50,0 % vs 11.1 %). La mortalidad global de los pacientes hospitalizados fue del 16,4 %. De los que recibieron Estreptoquinasa solo falleció el 10.9 %, mientras que el 26,0 % de los que no se trombolizaron murieron.
