Pseudoaneurysm of the left brachial artery. Case report

Introduction: complications of vascular access are an important reason for hospital admissions for patients on dialysis. These complications can sometimes aggravate the condition of the nephropathic patient and lead to death due to the exhaustion of vessels for the hemodialysis procedure.
Objective: to present the surgical treatment performed on a patient with a rare pseudoaneurysm.
Case presentation: female patient with a history of chronic renal failure of unestablished cause and on hemodialysis regimen. She was received in the Angiology room and admitted. Several complementary tests were performed, of which the most relevant was the color Doppler ultrasound, which showed loss of continuity of the wall of the left brachial artery, with a pulsating hematoma at that level that contained the arterial blood flow with the Ying-Yang sign. It was decided to resect 5 centimeters of the artery and interpose an autologous graft consisting of a fragment of the internal saphenous vein of the right lower limb. Terminal anastomosis was performed with 5/0 Prolene non-absorbable vascular suture.
Conclusions: with the surgical technique carried out, it was possible to maintain adequate arterial flow, the arterial irrigation of the affected upper limb was restored and major amputation was avoided.
