Trombofilia hereditaria en la enfermedad tromboembólica venosa
Abstract
La trombosis venosa es una enfermedad común, con una incidencia anual de 1/1000 y compleja, definida por múltiples interacciones entre componentes genéticos y del entorno(1,2). La trombofilia hereditaria se define como la tendencia genéticamente determinada al tromboembolismo venoso que ocurre de manera espontánea a edad temprana (antes de los 40 años) sin causa aparente y con tendencia a recurrir. Las trombosis suelen manifestarse en sitios poco comunes(3). Se realiza una revisión bibliográfica de las trombofilias hereditarias y su relación con la enfermedad tromboembólica venosa. En la introducción abordamos los aspectos generales; en el desarrollo nos referimos a la clasificación. Se hace mención a elementos importantes del síndrome de antitrombina III, déficit de proteína C y S, resistencia a la proteína C activada o mutación Leiden del factor V, mutación G20210 de la protrombina y aumento de la concentración del factor VIII. Se hace referencia al diagnóstico y se exponen las consideraciones terapéuticas. Se concluye anunciando que la trombofilia es un complejo de estado de alteración genética múltiple pues cada día se conocen nuevas mutaciones genéticas que dan lugar, de forma aislada o en sorprendentes combinaciones a estados congénitos de hipercoagulabilidad o trombofilia.Downloads
