Sialadenitis with crystalloid formations. Case report
Abstract
Introduction: the presence of crystalline structures in the cytological smears of the salivary glands is not a common finding; these may have a very different morphology –from needles and rods to different sizes plaques– and suggest the presence of tumor or not tumor diseases in the salivary glands.
Objective: to present a case that, because of its complexity, serves to highlight the importance of clinical-imagenological and cytological correlation in diagnosing the disease.
Case presentation: a male patient, 55 years old, who attends to the Consultation of Maxillofacial Surgery of the General Teaching Hospital "Enrique Cabrera" in Havana, for presenting a tumoral mass with three months of evolution, in the left parotid gland. The patient reported slight pain during the digital pressure of the area and it was observed the output of clear saliva by the gland excretory duct. In the ultrasonographic study was found a rounded formation of 29,8 mm by 23,1 mm with central echolucent area on the left parotid gland. The fine needle aspiration resulted in the presence of several crystalloid formations eosinophilic, not refracting, of rectangular appearance and varying sizes, intermixed with acute inflammatory elements. The diagnosis was sialadenitis with presence of crystalloid bodies. Treatment with antimicrobials was indicated and the clinical picture decreased.
Conclusions: should not pose a definitive diagnosis based on the presence of these crystals regardless of the proper relationship of clinical, imaging and cytological methods, to guide conservative treatment and avoid unnecessary surgical operation.
