Adverse reactions of intrathecal morphine in elective cesarean section

Introduction: adequate control of postoperative pain is one of the fundamental pillars of the current anesthetic procedure. Morphine can produce adverse effects that must be considered promptly.
Objectives: to evaluate the safety of postoperative analgesia and identify adverse reactions in patients undergoing cesarean section, treated with intrathecal morphine in the obstetric unit of the “Vladimir Ilich Lenin” General University Hospital in Holguin province.
Methods: quasi-experimental, prospective and longitudinal study. 60 patients were analyzed, using non-probabilistic sampling, from a universe of 362 with elective cesarean section from July to December 2022. They were administered intrathecal morphine (0.2 mg) plus hyperbaric bupivacaine (7.5 mg). Demographic variables, pain intensity using a visual analogue scale, and adverse effects (nausea, vomiting, pruritus, respiratory depression and others) were recorded. It used the descriptive statistic; and the ethical principles were met.
Results: during the first 24 postoperative hours, 28.33 % of the patients had no pain, 68.33 % had mild pain, and 3.34 % had moderate pain; no acute pain was reported. Adverse reactions occurred in 58.33 % of the sample; the most obvious was pruritus (48.33 %), followed by nausea (28.33 %), and vomiting (15.00 %). No patient presented respiratory depression or other adverse events of interest.
Conclusions: the administration of intrathecal morphine in doses of 0.2 mg constitutes an adequate and safe option for postoperative analgesia in patients undergoing cesarean section. Its adverse reactions are mild.
