Atrioventricular block in a COVID-19 positive pregnant woman. Case report

Abstract

Introduction: the pandemic generated by the SARS-CoV-2 coronavirus was a litmus test for the medical community due to its magnitude, extension, severity, morbidity and mortality, different forms of presentation and symptoms that were sometimes unrelated to what was known and made difficult to diagnose.

Objective: to present a case of second degree atrioventricular block, Wenckebachen type in a COVID-19 positive pregnant woman.

Case presentation: 25-year-old patient, with apparent health history, obstetric history of three pregnancies, two births, no abortions; six-day hospital admission by risk percentile. At 37.6 weeks, a second degree atrioventricular block, Wenckebach type, was diagnosed; He also presented respiratory manifestations lasting four days. It was decided to place a temporary pacemaker. A rapid test and a PCR (polymerase chain reaction) test were performed, which were positive. Symptomatic treatment was instituted. On the fifth day of evolution, evolutionary PCR was performed with a negative result and she was transferred to an open room, with sinus rhythm and completely asymptomatic.

Conclusions: The systemic manifestations related to COVID-19 were varied and often complicated the diagnostic process, including cardiovascular manifestations and, in particular, arrhythmias.
