Reevaluación diagnóstica de una muestra de pacientes del Hospital de Morón con prolapso valvular mitral.
Abstract
Introducción: El prolapso de la válvula mitral (PVM), es la enfermedad valvular cardíaca más frecuente en la población general y se encuentra mayormente en personas jóvenes, del sexo femenino. Método: El universo de estudio estuvo conformado por 48 pacientes, procedentes del Hospital Universitario de Morón con el diagnóstico de PVM, que asistieron a la consulta de ecocardiografía del Cardiocentro de Santa Clara. A todos se les realizó ecocardiograma en reposo con un equipo Aloka 5500 con el objetivo de redefinir el diagnóstico. Resultados: Solo el 18.8% de los pacientes tenía PVM clásico y el 22.9%, no clásico. 30 pacientes pertenecían al sexo femenino y 37 eran blancos. Los pacientes sin PVM presentaron mayor número de síntomas: ansiedad (89.3%), disnea (64.3%), dolor torácico (60.7%), entre otros. En los pacientes con PVM las palpitaciones constituyeron el síntoma predominante (100% clásico y 72.7% no clásico), seguido del dolor torácico (88.9% y 63.6% respectivamente). Todos los pacientes con PVM presentaron soplo sistólico y chasquido, aunque no todos mesosistólicos; en cambio, solo el 57.1% de los pacientes sin PVM presentaba soplo sistólico y el 25.0%, chasquido mesosistólico. El 55.6% de los pacientes con PVM clásico presentaba regurgitación mitral muy ligera y el 82.1% de los pacientes sin PVM no tenía regurgitación. Conclusiones: El 58.3% de los pacientes no tenía PVM. Aquellos que lo padecían eran predominantemente blancos, del sexo femenino y se quejaban de palpitaciones y dolor en el pecho; presentaban soplo sistólico apical y chasquido y el grado de regurgitación fue, principalmente, muy ligero.
