Hemorragia subaracnoidea. Comportamiento y manejo clínico en las unidades de atención al grave
Abstract
Se realizo un estudio descriptivo lineal de 36 pacientes portadores de Hemorragia Subaracnoidea (HSA) admitidos en las salas de atención al grave del Hospital Provincial Docente “Dr. Antonio Luaces Iraola” de Ciego de Avila, en un período de un año. Predominó el sexo femenino y los mayores de 41 años. La HSA espontánea y dentro de estas, las angiopatías fueron las causas más frecuentemente encontradas. El diagnóstico fue realizado clínicamente en el total de los casos, en el total de los casos, en el 97.22% de los mismos se utilizó la punción lumbar como proceder diagnostico y solo en un 2.77% nos auxiliamos de la TAC de cráneo simple. La mayor parte de los pacientes tenían a su ingreso un estado clínico aceptable, encontrándose entre los estadíos I y III de las escalas de Hunt y Hess y la WFNS. Solo 1/3 de los casos mostraron a su ingreso signos y síntomas clínicos neurológicos de lesión encefálica severa. Todos los pacientes con el diagnóstico de HSA fueron admitidos en una sala de atención al grave y casi en su totalidad, en las primeras 24 horas que siguieron al ictus. La terapéutica médica establecida fue adecuada en todos los casos. Como complicaciones más frecuentes se encontraron el resangramiento, el vasoespasmo y la sepsis respiratoria. 1/4 de los enfermos fallecieron, encontrándose dentro del 44.44% de los pacientes que no fueron remitidos a un servicio Neuroquirúrgico para su estudio. Solo el 55.56% de los encuestados fue remitido a un servicio neuroquirúrgico y de estas remisiones, el 2.77% fue realizada en las primeras 72 horas.
