Predictores de mortalidad en el manejo de las peritonitis graves
Abstract
Objetivo: Determinar cuales son los predictores de la mortalidad en el manejo de los pacientes conperitonitis graves en nuestra institución. Método: Se realizó un estudio observacional retrospectivo con
107 pacientes en un período de 10 años. A los primeros 25 se les aplicó la técnica del abdomen abierto,
35 fueron tratados con Relaparotomías programadas, 20 reparación abdominal por etapas y en 27 se
utilizó Relaparotomía a demanda, se estudiaron los parámetros: Edad, sexo, puntaje APACHE II, PIA,
enfermedades asociadas, causa de muerte, complicaciones postoperatorias, técnica quirúrgica
empleada y tiempo de inicio de la reoperación. Se realizó un análisis de regresión logística para
determinar los verdaderos predictores. Resultados: El grupo con puntuación APACHE II de 21.42 ±
3.24, presentó como enfermedades asociadas neumopatías crónicas (14.95%), hipertensión arterial
(13.08%) y diabetes Mellitus 12 pacientes, la media de valores de presión intrabdominal fue de 26 ±
3.24. El SDMO constituyó el 40.18% de las complicaciones y la mortalidad fue del 38.31%, el 80.48%
falleció cuando la reoperación comenzó después de las 36 horas. Conclusiones: Se concluyó que los
factores que predijeron mortalidad en nuestra serie fueron: APACHE II ≥18, presencia de SDOM,
presencia de complicaciones postoperatorias, existencia de enfermedades asociadas, cifras de PIA>20
y el inicio de la primera reoperación después de las 36 horas, con p<0.05.
