Comportamiento de los pacientes quirúrgicos en los servicios de cuidados intensivos e intermedio del hospital provincial de Ciego de Ávila. Año 2006.
Abstract
Se realizó un estudio observacional descriptivo prospectivo que incluyó 80 pacientes quirúrgicos que fueron ingresados en el Servicio de Cuidados Intensivos del Hospital Provincial "Dr. Antonio Luaces Iraola" de Ciego de Ávila durante el año 2006. Predominó el sexo femenino y las edades comprendidas entre 50–59 años y de 70–79 años. El mayor porciento (53.8 %) de los pacientes ingresados fueron recibidos de forma directa sin previa remisión por otros niveles de atención. El tiempo entre el inicio de los síntomas, diagnóstico, tratamiento – quirúrgico y la mortalidad de los pacientes tiene una relación directamente proporcional, ya que cuando éste aumentó provocó un incremento de la mortalidad. La correlación entre el diagnóstico preoperatorio y definitivo fue del 87.5 %. Los diagnósticos más frecuentes fueron Síndrome Tumoral (36.3 %), Síndrome Peritoneal (32.5 %) y Fractura Ósea (10.0 %), correspondiendo también los mayores porcientos de fallecidos a estos diagnósticos (10.0 %, 8.8 % y 7.5 %) respectivamente. Las principales complicaciones fueron la Bronconeumonía (23.3 %), Shock Séptico (20.5 %) y la Peritonitis (15.1 %). La principal causa de muerte fue la Sepsis (FMO) con un 40.0 % y Bronconeumonía 36.7 %. Los antimicrobianos más utilizados fueron el Metronidazol, Cefalosporinas y Aminoglucósidos. Existió una correlación del 80 % de los pacientes fallecidos con tromboembolismo pulmonar entre el diagnóstico y causa directa de muerte.
