El dolor precordial en el niño: un reto en la atención médica de urgencia

Abstract

El dolor torácico de posible origen cardiaco en el niño es una causa frecuente de asistencia al Policlínico Principal de Urgencias, generando una larga y costosa evaluación diagnóstica, por ello se realizó un estudio observacional descriptivo en una muestra comprendida por 60 pacientes que acudieron aquejando dicho síntoma en el segundo semestre del año 1999 al PPU del municipio Ciro Redondo. Se diagnosticó anomalía cardiaca en el 16.7% de los niños estudiados, la presencia de antecedentes patológicos personales de enfermedad cardiaca, así como las características del dolor y los signos detectados en el examen físico, permitieron sugerir el diagnóstico de: Prolapso de Válvula Mitral (13.3%), Pericarditis Reumática y Estenosis Mitral Post Reumática (1.7%) respectivamente, en relación con el total de los casos estudiados. Estas impresiones diagnósticas se corroboraron por ecocardiografías, la radiografía de tórax fue positiva en un paciente y el electrocardiograma negativo en todos los cardiópatas. Las afecciones de origen no cardiaco más observadas fueron las correspondientes al sistema osteomioarticular y de la esfera psíquica. Se concluye que el dolor torácico en el niño rara vez tendrá origen cardiaco, y la anamnesis más un examen físico exhaustivo, son los principales medios diagnósticos para brindar adecuada atención a estos pacientes por parte del especialista en Medicina General Integral en el Policlínico Principal de Urgencias.
