Factores pronósticos del trauma craneoencefálico en el adulto mayor
Abstract
Introducción: El trauma cráneo encefálico en el adulto mayor en Camagüey tiene una incidencia que se acerca al 64%. Existe consenso unánime en la literatura en que la mortalidad por esta causa aumenta y el pronóstico es más desfavorable en los pacientes mayores de 60 años. Objetivo: Demostrar la posible influencia de factores de mal pronóstico del Trauma Cráneo Encefálico en el adulto mayor. Método: Se realizó un estudio descriptivo analítico en 240 pacientes de 60 años y más, que sufrieron alguna variedad de Trauma Cráneo Encefálico, que motivó su ingreso por el Servicio de Neurocirugía del Hospital Universitario “Manuel Ascunce Domenech” de Camagüey, durante los años 2004 a 2008. Se aplicó la prueba Chi cuadrado con un nivel de significación de 0,05. En caso de encontrarse un resultado significativo en esta prueba, se calculó la razón de riesgo que puede interpretarse como la probabilidad de fallecer o tener un pronóstico desfavorable cuando el factor está presente. Resultados: El riesgo en los pacientes más de 80 años es 5,62 veces mayor que en los menores de 80 años. La razón de riesgo asociada al Trauma Cráneo Encefálico grave es 441 veces mayor en comparación con otras variedades del trauma. La realización del tratamiento quirúrgico puede aumentar 67,5 veces la probabilidad de fallecer en estos pacientes. Conclusiones: La edad avanzada, el antecedente de enfermedad cardiovascular y la severidad del trauma, así como la presencia de hematomas intracraneales traumáticos, el desarrollo de complicaciones y el tratamiento quirúrgico incrementaron de manera significativa el riesgo de fallecerDownloads
