Neuro-ophthalmologic manifestations of spontaneous carotid-cavernous fistula. Case report

Introduction: carotid-cavernous fistula is an anomalous arteriovenous communication –rare– between the carotid artery and the cavernous sinus. It appears in an anterograde position to the orbit, and severely compromises the eyeball.
Objective: to describe the neuro-ophthalmological manifestations of a patient with spontaneous carotid-cavernous fistula.
Case presentation: female patient, 58 years old, with a history of decompensated arterial hypertension. She presented mild pain in her right eye, decreased vision, redness, ocular protrusion, and binocular double vision for 15 days. Through neuro-ophthalmological and neuroimaging examinations, a spontaneous carotid-cavernous fistula was diagnosed, which closed spontaneously with the indicated conservative treatment.
Conclusions: a thorough examination of the patient allowed us to identify the symptoms and signs, and the imaging characteristics of carotid-cavernous fistulas, as well as their differential diagnoses. Which is crucial to establish timely treatment, and avoid serious ocular and neurological complications.
