Dolor precordial típico en pacientes sin enfermedad coronaria.
Abstract
Se realizó una investigación clínica en 176 pacientes con dolor precordial sugestivo de Cardiopatía Isquémica en el Hospital Provincial de Morón entre septiembre de 1999 y agosto del 2000 para conocer las causas más frecuentes de dolor precordial de etiología no cardiovascular en pacientes con sospecha de Cardiopatía Isquémica. Se les realizaron los exámenes complementarios necesarios para definir la causa del dolor y solo 93 pacientes presentaron causa cardiovascular demostrable, los 83 restantes sufrían enfermedades biliodigestivas como litiasis vesicular (37.9%), reflujo gastroesofágico (14.9%), gastritis (13.8%), hernia hiatal (11.5%), colecistitis aguda (10.4%), úlcera péptica (8.1%) y pancreatitis aguda (3.4%). El 50.6% de los pacientes pertenece al sexo femenino, 67 (80.7%) son blancos y la edad media fue de 56 años. El complementario más útil para el diagnóstico fue la ecografía. Después del tratamiento se logró la remisión de los síntomas en 90.4% de los pacientes. Las enfermedades biliodigestivas encontradas en estos enfermos produjeron dolor precordial sugestivo de Cardiopatía Isquémica en el 30.6% del total de pacientes ingresados con ese diagnóstico. El dolor precordial, aunque a veces típico, no siempre es de causa coronaria.
