Potenciales evocados visuales a patrón empleando estímulos no convencionales en pacientes con neuropatía optica epidémica cubana en la provincia Ciego de Avila
Abstract
Se realizó un estudio experimental comparando dos formas de estimulación visual a pacientes con la Forma Óptica de la Neuropatía Epidémica Cubana (FONEC) en la provincia de Ciego de Ávila, para evaluar si el empleo de estímulos visuales dirigidos selectivamente al tipo de afectación neurooftalmológica incrementa la eficiencia diagnóstica del Potencial Evocado Visual a Patrón (PEV-P) en la FONEC. Se utilizaron dos condiciones de estimulación visual (CE): 1) Campo completo (16 x 12 grados sobre la retina) y Tamaño de Cuadrícula (TC) de 20’ de arco; 2) Campo completo y TC de 5’ de arco. Con un análisis de varianza se evaluó la influencia del grupo y el tamaño de cuadrícula sobre la latencia y amplitud de P100 en pacientes y controles. Los valores medios de latencia más prolongados se encontraron en los enfermos para las dos CE. Asimismo, se constataron valores medios menores para la amplitud. Se realiza también la clasificación individual de los potenciales tomando como criterios de anormalidad latencia del P100 mayor media más 2,5 desviación estándar del grupo control o ausencia del componente. El cálculo de la sensibilidad y del índice de verosimilitud positivo y negativo demostró que la CE donde se empleó TC de 5’ de arco aventajó a la CE en que se utilizó un TC de 20’ de arco. Nuestros resultados nos permitieron concluir que el empleo de estímulos visuales adecuados al tipo de lesión clínica incrementa la efectividad diagnóstica del PEV-P.
