Uso e indicación de agentes antimicrobianos en pacientes geriátricos
Abstract
Se realizó un estudio descriptivo longitudinal prospectivo que incluyó a 308 pacientes geriátricos que ingresaron en la Sala A del Servicio de Medicina Interna para apreciar el uso e indicación de antimicrobianos en el año 2003. Predominó el sexo femenino y la edad comprendida entre los 80 y 89 años. Los diagnósticos más frecuentes fueron la neumonía (74,4 %) y la gastroenterocolitis (10,7 %). Los antimicrobianos más empleados fueron las Penicilinas (36.3 %) y Cefalosporinas (14,0 %). Los mayores porcientos de pacientes 44,2 % y 36,0 % usaron 1 y 2 antimicrobianos respectivamente. Se detectaron algunas deficiencias tales como: el Pensamiento Clínico Epidemiológico expresado en las Historias Clínicas fue deficiente, principalmente cuando se realizan cambios de tratamientos; indicación de antimicrobianos por menos de 72 horas, pobre utilización del valor de la Creatinina para el cálculo de la dosificación de los antimicrobianos.
