Analgesia epidural continua postoperatoria en cirugía de prótesis total de cadera

Abstract

Se realizó un estudio preexperimental con la finalidad de evaluar la utilidad de la analgesia epidural continua postoperatoria en la cirugía de prótesis total de cadera con bupivacaína 0.5 %. Se estudiaron 24 pacientes a los cuales se le realizó cirugía protésica de cadera en el período de enero a diciembre 2005. El mayor número de paciente tuvo un tiempo prolongado de analgesia ( más de 5
horas) después de la administración de 5 ml de bupivacaína. En el momento de la administración de la analgesia, a las 6 horas el mayor número de paciente según la escala de dolor utilizada no tenía dolor o este era ligero, 3 pacientes tenían dolor moderado y dos tenían dolor intenso y excepto un caso todos aliviaron después de la administración del anestésico. A este caso se le administró AINES. La hipotensión arterial fue la complicación que con más frecuencia encontramos, la frecuencia respiratoria y cardiaca no sufrieron variaciones importantes, y se presentó un caso de hipertensión arterial ligera. No hubo otra complicación. La rehabilitación se inició a la mañana siguiente a la cirugía y de manera general se evalúa de muy satisfactoria para el colectivo médico y para los pacientes.
