Efficacy of subarachnoid anesthesia with hyperbaric bupivacaine and morphine in cesarean section

Introduction: spinal anesthesia with hyperbaric bupivacaine and low-dose morphine provides effective and safe analgesia during cesarean delivery.
Objective: to evaluate the effectiveness of anesthesia with 0.5% hyperbaric bupivacaine and intrathecal morphine in patients undergoing cesarean section.
Methods: a quasi-experimental study with a control group was conducted at the National Hospital “Guido Valadares,” Dili, Timor-Leste, between January and June 2024. The study included a population of 63 patients who underwent cesarean section under subarachnoid anesthesia. Variables assessed included age, gestational age; intraoperative vital parameters such as heart rate, mean arterial pressure, and peripheral oxygen saturation measured at the beginning and end of the operation; postoperative pain intensity; and complications. Ethical principles were observed.
Results: the mean age was 33.16 ± 5.10 years and gestational age was 38.8 ± 0.7 weeks. No statistical differences were observed between groups in vital parameters (p > 0.05), although in the study group the initial heart rate showed a significant difference (p < 0.05), indicating a more stable hemodynamic response. This group presented lower scores on the visual analog scale during the immediate postoperative period (p = 0.00), reflecting lower pain intensity. The most frequent complications were pruritus (20.60%) and nausea (20.60%), with no cases of respiratory depression in either group.
Conclusions: the combination of morphine and hyperbaric bupivacaine in subarachnoid anesthesia provides more effective postoperative analgesia compared to the use of bupivacaine alone.
