Anestesia para colecistectomía laparoscópica. Revisión del tema

Abstract

La cirugía de Invasión  mínima se ha popularizado en los últimos años con el advenimiento de las técnicas laparoscópicas, cuyas fronteras no solo se limitan a procedimientos ginecológicos, sino que se ha extendido en el campo de la Cirugía General, Ortopedia, Tórax y Urología. La anestesia general es el método de elección en estos pacientes, dado que permite al anestesiólogo un control preciso de la ventilación y modificar los parámetros ventilatorios con base en las alteraciones que puedan presentarse, brindándole una opción segura y eficaz. La colecistectomía laparoscópica impone nuevos retos al anestesiólogo, debiéndose  conocer en profundidad los cambios que ocurren a nivel hemodinámico y respiratorio a causa del neumoperitoneo establecido y a la influencia de los cambios de posición necesaria para realizar dicha técnica. El Dióxido de Carbono ha sido el gas de elección debido a que no es inflamable, tiene buena tolerancia por el paciente, alta difusibilidad y una rápida velocidad de eliminación pulmonar. Existen pocas contraindicaciones de esta técnica y  absolutas serán, los paciente con colapso cardiovascular grave y paciente con compromiso de la presión intracraneal.
