Morbimortalidad por sangramiento digestivo alto en el adulto mayor. Análisis de 3 años
Abstract
Con la finalidad de conocer la morbimortalidad por sangramiento digestivo alto del adulto mayor enel Hospital Provincial Docente “Dr. Antonio Luaces Iraola” de Ciego de Ávila, se realizó un estudio
descriptivo de corte transversal, en el período de Enero de 2005 a Diciembre de 2007. El universo
incluyó los pacientes ingresados con diagnóstico de sangramiento digestivo alto con 60 años o más.
Los resultados muestran que el grupo de edad más afectado fue el de 60 a 74 años y el sexo masculino
el de mayor morbilidad. La úlcera gástrica predominó sobre el resto de las causas (41,5%). El
tratamiento médico fue empleado en mayor número de casos que el tratamiento quirúrgico. La
gastrostomía con ligadura del vaso sangrante fue la técnica quirúrgica más empleada (52,5%). El grupo
de 75 años y más presentó mayor mortalidad (56,2%), con un predominio del sexo masculino, siendo
más elevada en aquellos pacientes que recibieron tratamiento quirúrgico (42,1%). La patología
asociada a mayor mortalidad fue la úlcera gástrica (43,7%) y la hipovolemia la principal causa de
muerte (62,5%). Estos resultados demuestran que el sangramiento digestivo alto continúa siendo un
problema de salud de difícil manejo en los servicios de urgencias y quirúrgicos de nuestro hospital
